Polygraph Lie Detector Reports
The final report of any lie detector test is arguably the “product” that a client receives following the exam itself. If prepared correctly, this document should provide a client with a clear understanding of the test issues, disclosures, and analysis of the data collected during the procedure. Any Examiner who does not commit to his/her test results by providing a written report should not be trusted. Here are the elements that every report should include, noting that the report “style” will vary from examiner to examiner.
REQUIRED ELEMENTS OF A POLYGRAPH REPORT
- Name of the person tested (Examinee)
- Date of the Exam
- Purpose of the Exam
- Any Admissions or Statements made by the Examinee
- List of Relevant Questions Asked – See our Page about Question Rules
- Answers Given to the Relevant Questions (“yes” or “no” only)
- Definitions of any ambiguous Terms used in the Questions
- Copy of Examinee’s signed and dated written statement, if the exam is of the “Statement Verification” format.
- Results of Chart Analysis (TDA) – See our Page explaining Test Results
- Name of Examiner (and signature, if format allows)
- Examiner Contact Information

- Type of Equipment Used
- Location of Exam
- Testing Format Used
- Examiner Curriculum Vitae (CV)
- Printout of Manual Score Sheet (Primary / Official Result)
- Printout of Computerized Score Sheet (Secondary Result)
- Copy of polygraph charts **
- Audio or Video recording of exam **

HOW LONG DOES IT TAKE TO GET THE REPORT?
Most examiners will provide a detailed written report within a few days of the exam. There are several factors that can affect this time frame. Keep in mind that if the examiner is traveling extensively (as many do) the report can not be prepared until the examiner returns to his/her office.
